We offer competitive pay and comprehensive benefits including Paid Time Off & 401(k) Responsibilities Receive and process new patients for admission to our agency using EMR system.
Coordinates care by assigning case manager and clinician(s) to ensure 48-hour admission.
Communicates with various healthcare professionals to provide highest level of care.
Use HIPAA compliant processes to receive and process orders/faxes for new and current patients.
Performs follow up calls to patients and caregivers to offer clear and supportive communication.
Performs other various coordination duties to support day to day work flow.
Qualifications Graduate of an approved Nursing education program.
Current, active license to practice as LPN, with at least 2 years of successful experience in health care (preferably home care), practicing as an LPN.
Demonstrates excellent oral and written communication skills and problem-solving capabilities.
Must possess and maintain current, valid driver's license, reliable automobile transportation, proof of current automobile insurance, current CPR certification by either American Heart Association or Red Cross.
Basic computer skills and office technology required.
Possesses great attention to detail, accuracy, and ability to complete assignments in an efficient and timely manner.
